Wear loose clothing, bring prescription glasses if worn (remove contact lenses) / Bring information of any prescription Medication you are currently taking, bring your hearing aids if worn
Medical Conditions
If you have any pre-existing medical conditions such as diabetes (type 1 or 2), high blood pressure, high cholesterol, thyroid disorders, epilepsy, mental health conditions or any cardiovascular disease you must bring a letter from your doctor which outlines the below:
Diagnosis and date of onset
Confirms you are being treated for the condition (provide management plan)
Confirms condition is under control (provide pathology results if applicable)
Medication you are currently taking and potential side effects
Outlines any restrictions that may impact your ability to work
Failure to provide this information will result in a delay in the completion of your medical. Please ensure the GP letter is dated within 3 months of this medical
Audio
Please ensure you are not exposed to more than 80dB for 16 hours prior to your medical. This is approximately equivalent to the noise of a vacuum cleaner one metre away or being curb side in heavy traffic. Loud music should be avoided.
Spiro
No smoking 1 hour prior to assessment / if you are asthmatic bring your inhaler/Puffer with you
Drug and Alcohol Testing (DAS)
Avoid, excessive water consumption, Poppy Seeds, Gum, Mouth Wash & caffeinated drinks such as coffee, coke etc prior to your assessment
Functional
Must wear enclosed shoes and exercise clothing. Please bring a towel and water bottle.
Respiratory Fit Test
Please ensure you are clean shaven to ensure the mask can be adequately fitted
ECG
Please ensure thick chest hair is shaved off prior to your appointment, to ensure correct ECG reading
Flu Symptoms
If you present to the clinic with flu-like symptoms the appointment may be cancelled and a Fail to Attend Fee will apply
Late Arrivals
Will only be seen at clinic discretion
Urine Sample
You may be required to provide a urine sample for the analysis of sugar levels or as part of a drug screening
Photo ID
PHOTO ID MUST BE PRESENTED TO THE ASSESSOR – Failure to produce adequate identification may result in parts of your assessment being reported as invalid, requiring them to be redone.
